Developmental Disabilities

Case Management is designed to help individuals locate needed services.  CCIL Social Workers assist by coordinating the services, which can be obtained through either the Medicaid Targeted Case Management Program or the Medicaid Title XIX MR/DD Waiver Program.  Case Management can also be called Service Coordination.

To receive services, an individual must:

v      be at least two years of age

v      be Medicaid eligible or be able to privately pay for services

v      be diagnosed with an eligible diagnosis (mental retardation or developmental disability)

v      be in need of case management services

Referrals can begin with a phone call to any area CCIL office.  Referrals can also be made in person or in writing. Individuals can refer themselves, family members, or friends. Any person having responsibility for another individual can make a referral for service.

DD Case Management assists individuals in gaining access to needed medical, behavioral health, social, educational and other services.  This goal is accomplished through a number of activities that the Social Worker performs.

These activities include:

ü       Assessment

ü       Service Planning

ü       Referral and  Linkage

ü       Advocacy

ü       Crisis Response Planning

ü       Service Plan Evaluation

DD Case Management is the best choice for you because:

Ø      CCIL assigns you a Social Worker who will develop a professional relationship with you

Ø      CCIL Social Workers carry a small, manageable case load which permits time for the personal service you need

Ø      CCIL Social Workers are licensed by the West Virginia Board of Social Work Examiners

Ø      CCIL provides 24 hour crisis on-call services

Ø      CCIL Social Workers will make monthly visits to make sure you have all the services you need

Ø      CCIL believes in doing one thing well, case management, rather than trying to be all things to all people
