AFFORDABLE INDEPENDENCE

The Care Management Services Program is designed to act as an affordable long-term health care alternative for West Virginia residents who would normally be placed in a nursing home or to provide temporary assistance for those recuperating from an illness, injury or surgery.  Clients remain in their own homes supported by a package of services that is custom designed to assist them in maintaining their independence for as long as possible.   Services are contracted, coordinated, monitored and managed by an experienced professional care management team consisting of West Virginia Licensed Social Workers, Registered Nurses and knowledgeable administrative and support personnel.
