ADMISSION/REFERRAL PROCESS

Admission into WV Select’s Care Management Services Program is open to any individual evidencing a need for long- or short-term care and meeting basic medical and financial criteria.  Referrals can be made in person at any WV Select office or by calling your local WV Select office listed on the back of this brochure.
